As part of our ongoing commitment to integrated care, our Support Coordinator and Programmes Lead participated in an information-sharing meeting with over 35 clinicians at the Royal Hospital in Donnybrook Dublin 4.
The meeting provided a valuable opportunity to share information about Making Connections supports and hear directly from clinicians about the experiences, challenges, and needs they are seeing among their clients.
Our main partners are the HSE, and we work closely with them to ensure our services complement the wider health and social care system. This collaboration at Donnybrook Hospital was a great example of integrated care in action. By bringing together community supports and clinical expertise, we can identify opportunities for earlier intervention, strengthen connections, and contribute to preventative care — something that is essential in supporting older people to maintain their health, independence, and quality of life.
